The instructions below are to help guide international students through the process of navigating to and submitting an Information Release Authorization Form.

Navigation Instructions:
Upon login (https://stcloudstate-isss.terradotta.com), the landing page will display your personal information at the top, which remains the same throughout the portal. The lower portion of the page will change as you move between the three main sections:
Personal & Program
Includes general personal information, academic details, contact information, and dependent information (if applicable).
Control Center
Shows a summary of your requests—Ongoing requests, Completed requests, Documents, CPT summary, OPT summary, and Available Requests.

Important Updates
Announcements pertaining specifically to you.

Control Center
Summary of requests- Ongoing requests, Completed requests, Documents, Curricular Practical Training (CPT) summary, Optional Practical Training (OPT) summary, and Available requests.

Information Release Authorization Form Instructions:
Select Information Release Authorization Form
From the menu of available request options, select “Information Release Authorization Form.”
This form allows you to authorize the Center for International Studies (CIS) to communicate with a specific individual or agency regarding your student records, application information, or immigration-related matters.
Once selected, review the instructions and proceed to the next section of the form.

Information to Be Provided (Part 1)

- Choose the appropriate category for the office or department relevant to your authorization (International Admission, International Student & Scholar Services, or Study Abroad).
- Select the option that best matches the type of communication you want CIS to authorize.
- Click Next to continue.

Section 1 – Information to Be Provided (Part 2)

- Provide the Family/Last Name of the individual or agency you want to authorize.
- Provide the Given/First Name or the official agency name if applicable.
- Indicate the individual’s relationship to you (parent, friend, agent, etc.).
- Enter the email address of the authorized person or agency.
- Enter the cell phone number of the authorized person.
- Provide the full address of the authorized individual or agency, including city, state/province, country, and zip code.
- Click Next to proceed.

Section 2 – Authorization Options

Select the type of information you authorize CIS to share:
- Communication related to admission decisions or your application.
- Account information, student conduct information, health and safety matters.
- Academic or immigration-related matters.
You may check more than one box depending on what you want to authorize.
Click Next to continue.

Section 3 – Additional Permissions & Final Confirmations

Review each statement carefully and check the box to indicate your acknowledgment:
- Waive privacy rights under FERPA as needed for CIS to communicate with the authorized person.
- Permit CIS to share or release copies of documents after they have been processed (with exceptions such as TOEFL/IELTS original scores).
- Acknowledge that the authorization stays active until revoked in writing.
- Confirm that all information you provided is complete and accurate.
Once all required checkboxes are selected, click Submit, or choose Save as Draft if you need to return later.

Information Release Authorization Form – Student Instructions
Select Information Release Authorization Form
From the menu of available requests, select “Information Release Authorization Form.”
Review the instructions at the top of the page before beginning.
• All fields marked with * are required.
• If you need to stop at any time, click Save as Draft. Your draft will appear under Ongoing Requests, and you may return to it later.
Click Submit after reviewing the instructions to proceed with the form.
Section 1 – Authorization Information (Questionnaire)
In this section, provide the required information about the individual or agency you wish to authorize.
• Select the appropriate category from the dropdown (International Admission, International Student and Scholar Services, or Study Abroad).
• Enter the family/last name of the authorized individual or the official name of the agency.
• Enter the given/first name of the authorized individual, or the agency name if applicable.
• Specify your relationship to the authorized person (parent, friend, agent, etc.).
• Provide the email address of the authorized person.
• Provide the cell phone number of the authorized person.
• Enter the complete address of the authorized person, including city, state/province, country, and postal code.
After completing all required fields, click Next to continue.
Section 2 – Information You Authorize CIS to Share
Review the communication options and select the items you authorize CIS to discuss with the listed individual or agency.
• You may choose to authorize communication related to admission and application decisions.
• You may allow the release of account information, conduct issues, health and safety concerns.
• You may authorize academic or immigration-related information to be shared.
Select all applicable checkboxes, then click Next.
Section 3 – Privacy Waivers and Acknowledgements
Carefully review each statement and select the required confirmations.
• Acknowledge the waiver of any privacy rights under FERPA, understanding communication may occur before, during, or after your program.
• Authorize CIS to share copies of documents after processing (note: original TOEFL/IELTS scores cannot be released).
• Confirm that the authorization remains active until you revoke it in writing.
• Declare that the information you provided in this form is correct, complete, and truthful.
Check all required boxes, then click Next or proceed to the final step.
Final Step – Submit the Authorization Form
After reviewing all sections for accuracy, select Submit to officially submit your Information Release Authorization Form to CIS.
If you wish to complete the form later, click Save as Draft.
It will take 5–7 business days to process your Information Release Authorization Form. Please monitor your St. Cloud State University email for any updates or requests for additional information from the Center for International Studies (CIS). Processing times may take longer during peak periods. If you have not received a response after 7 business days, and you have confirmed that there are no new messages in your SCSU email, you may email international@stcloudstate.edu to follow up on your request. Once processed, the authorization will remain active until you revoke it in writing.
