Instructions for academic advisors on processing program extension requests from students.
Instructions
The Academic Advisor will be asked to assist with a Program Extension to provide the student with more time to complete their program. The student provides the contact information for the Academic Advisor in the Program Extension request. The advisor will receive an email invitation to complete the approval form with Subject line: Request Advisor Notification.
Click on “here” to enter the request.

The form will have two main components. The top portion will not change. It provides general information about the student: Name, Program Dates, Level (Undergraduate/Graduate) and Major (Academic Program).
The rest of the form will consist of other portions that will help the Academic Advisor make a decision:
- Student submission- Information the student has provided CIS.
- Questionnaire- Questions/information for the Academic Advisor to help CIS make an informed decision as to approve or deny the request.
