HuskyTech cannot perform any work on your computer until you have read, acknowledged, and agreed to the following conditions.
In exchange for St. Cloud State University Technology Support Technicians providing service on this device, I confirm the following:
I am requesting services from HuskyTech to repair a potential issue with my device and authorize HuskyTech technician(s) to perform work on my device within their scope of service.
I own this device, and I understand technicians are not licensed service dealers.
I understand the work may change my device’s configuration.
I accept all risks, including but not limited to data loss, software or operating system malfunction or hardware failure; and I agree to release, indemnify and hold harmless, St. Cloud State University, ITS employees and agents from any and all claims even if caused by negligence of St. Cloud State University.
I understand there is no guarantee of outcome to the service.
I understand that if the device is checked in for service, there is a 30-day data storage limit. If the device is checked in for warranty repairs, there is a 90-day data storage limit.
I have read the Authorization/Indemnity Agreement. By clicking "I Agree" and submitting this service request, I understand that if I do not pick up my device in 90 days, the device will be subject to disposal according to SCSU guidelines and any associated data will be removed from HuskyTech storage.
